Monthly rent in our Memory Care community includes a broad range of personal care services that ensure individualized care, comfort, and attention. In addition to the services offered in our Transitions™ Care program, we can also provide assistance with mobility, bathing, dressing, and incontinence care.
Transitions™ Care
Our Transitions™ program is flexible in that it is designed to meet the ever-evolving personal care needs of those displaying the initial signs of memory loss. As one’s needs become more advanced, so does the level of care and attention we provide.
Our unique program allows individuals with like needs to live together, which helps residents to maintain their highest level of independence and interpersonal relationships.
Our community consists of single-story residences designed for those who have difficulties with the activities of daily living due to memory loss. Each of our individual cottages feature home-like interiors that include family-style dining rooms, living spaces, and inviting patios that are enclosed for experiencing the outdoors safely. In addition, our activity programming is tailored to meet the changing needs of each of our residents.
